If you have a scheduled appointment with Dr. Wenrick, read this page in its entirety. It has important information that you need to know prior to your arrival.
Office-Patient Agreements
APPOINTMENT
PREPAREDNESS
Paperwork:
You must arrive at your first appointment fully prepared with the entire registration packet filled out. Your appointment may need to be rescheduled if your paperwork is not completed prior to your scheduled arrival time.
Relevant Records and Health History:
1) Provide a summary of your symptom history in chronological order (onset, how long you have been dealing with it, progression).
2) List all previous trauma you have had, and their treatments with their results in chronological order. Regardless of whether you feel the trauma has impacted your TMJ symptoms, they all have importance when it comes to piecing the puzzle together.
3) List all current medications, dosage, how long you have been taking them, and why.
4) Bring copies of previous X-rays, CT, or MRI scans of your TMJ or bite.
5) Bring ALL dental splints that you may have worn in the past or are currently wearing.
6) If diagnosed with Sleep Apnea, bring all sleep study reports with you, along with your CPAP mask.
7) Bring your referral with you.
BROKEN/CANCELLED
APPOINTMENTS
ALL CANCELLED, MISSED, OR RESCHEDULED APPOINTMENTS WILL RESULT IN THE LOSS OF ANY AND ALL DEPOSITS OR PAYMENTS MADE TOWARD ANY SERVICE AND/OR ANY SCHEDULED SERVICE INCLUDING BUT NOT LIMITED TO EXAMINATIONS, CONSULTATIONS, AND/OR ANY TYPE OF TREATMENT BEING RENDERED.
OUR OFFICE DOES NOT GIVE REFUNDS FOR ANY DEPOSITS MADE. WE DO NOT GIVE REFUNDS FOR ANY TYPE OF SERVICE.
PAYMENT POLICY
Fee for Service:
Payment is required at the time of service, meaning that we do not accept or file with your insurance plan and you must pay in full for all appointments at Dr. Wenrick’s office. The office will let you know the cost of your appointment before scheduling you.
Insurance:
We have no relationship with your insurance company. If you would like to attempt to get reimbursed by your insurance company, please call them, and ask what you need to do to request reimbursement. The office will be happy to provide a Superbill showing that you paid the office in full. If you have any questions about insurance coverage, direct your questions to your insurance company.

PHONES
No Cell Phones in the Operatory:
It is important to turn off your cell phone before Dr. Wenrick takes you back for your appointment. This will allow for limited distraction during your time with Dr. Wenrick. If you are distracted by your cell phone during the appointment, you may be dismissed. If you have an emergency that requires your cell phone to be on, let us know.
